Revitive Circulation Booster

1. Title
The customer agrees the rented equipment is fully owned by Stouffville Pharmasave. This agreement allows the customer to use the equipment as it was manufactured for and in a safe manner. This is an agreement strictly between the customer and Stouffville Pharmasave and is not intended for subletting or shipping to an alternative user.

2. Physical Condition
The customer agrees that at the start of the rental period, the equipment is in good working condition, except for any defect noted on the equipment list. It is the customer’s responsibility to return the rental equipment to Stouffville Pharmasave in the same working condition.

3. Loss or Damage
The customer agrees to use the equipment at their own risk and agrees to indemnify and hold Stouffville Pharmasave harmless from damage, loss, liability, cause of action, claims or judgements from any injury or death incurred while in agreement, to property or person while property was in use or transportation.

4. Warranties & Representation
The customer agrees that Stouffville Pharmasave has provided adequate instruction of the safe and proper use of the equipment. The customer also agrees to use the rental equipment within the limitations as outlined by the equipment manufacturer itself.

5. Equipment Failure or Repair
The customer agrees to discontinue use of the equipment should it become unsafe or require repair. The customer also agrees to immediately notify Stouffville Pharmasave of the details of this occurrence. Stouffville Pharmasave will, at its own discretion, remedy the equipment issue within a reasonable period of time. Stouffville Pharmasave’s decisions will not relieve the customer of other sections of this agreement.

6. Failure to Return Equipment
Failure to return equipment will result in Stouffville Pharmasave billing for another period as defined by the start and end dates, unless notice is given prior to the agreement end date, a renewal bill is charged and the equipment can be collected by a representative or agent of Stouffville Pharmasave.

7. Payment & Charges
The customer agrees to pay the amount stated on the rental agreement plus applicable taxes. Additional amounts may be charged to the customer for the following; a) interest on overdue accounts is charged 1.5%/month or 18%/year, b) charges or fees from a collection process will be the responsibility of the customer, c) charges for replacement/repair/cleaning if restoration is necessary. The customer agrees to allow Stouffville Pharmasave to charge to customers’ credit card number for all charges, as outlined in this agreement.

8. Equipment Return
The equipment is rented to the customer for a set period as indicated on the rental form, any extension of this period must be approved by Stouffville Pharmasave, and these terms & conditions will apply to the extension. If the rental period has not been extended and is overdue, Stouffville Pharmasave may take action as permitted by law to retake the rental equipment and charge the customer for costs incurred in doing so. Equipment returned in a compromised state will result in the customer being charged a repair/cleaning fee, at the sole discretion of Stouffville Pharmasave.
